Your first visit to our office
When you walk through the doors of Comfy Smile Dental for the first time, you’ll be immediately welcomed as part of our patient family. We will establish the foundation of our relationship with you, and you will also get familiarized with our caring staff. During your first visit, we will obtain important general and personal information as well as your medical and dental history. We will obtain digital x rays of your teeth and will undergo a thorough examination of your mouth. Afterward, we will inform you of all the findings and create a personalized treatment plan that will address your dental needs.
You will be required to fill the following forms at your first visit. To expedite your waiting time you are welcomed to print these forms and bring them with you to our office.

Why I don't carry dental insurance?
Dental insurance isn’t a requirement to get treatment at our practice. It’s merely a convenience that allows many patients to reduce the overall cost of their procedure. If you don’t have insurance, you’ll still have access to all the same options as our insured patients — it may just cost you more out of pocket. You can also ask our staff about flexible payment plan options.
Will my dental insurance pay for my treatment?
Unfortunately, dental insurance won’t fully cover every treatment at our Davie, FL practice. In fact, some procedures may not be covered at all depending on the terms of your plan. The amount of payment you receive for any given treatment will vary depending on your insurance provider.

Do you offer interest free financing?
Yes! Our practice is proud to work with CareCredit, the most popular patient financing program in America. This convenient third-party financer allows you to pay for your dental care in installments over an extended period of time, easing the pressure of payment.
Best of all, you won’t be charged any interest if you keep up with your monthly payments and cover the full cost of treatment within 24 months. Low-interest plans are also available with extended repayment windows of up to 60 months.
